Roles & Responsibilities

• Provide high-quality outpatient physical therapy evaluations and individualized treatment plans
• Deliver evidence-based interventions including therapeutic exercise, manual therapy, and patient education
• Document assessments, plans, and progress thoroughly and in a timely manner
• Collaborate with clinic staff and interdisciplinary teams to coordinate patient care
• Ensure provider enrollment is completed prior to seeing patients and coordinate with credentialing as needed
• Maintain current licensure and certifications and support clinic quality and compliance initiatives

Qualifications

• Minimum 2 years of current outpatient physical therapy experience (required)
• Active New Hampshire Physical Therapy license (Compact allowed)
• Active American Heart Association BLS (required)
• Provider enrollment must be completed prior to patient care (credentialing handled by NCH but allow time for approval)
• Previous travel experience highly preferred
• Submission package items: work history, physical therapy skills checklist, supervisory-signed reference from the past year, license verification, permanent address and RTO included at time of submission

Knowledge & Skills

• Strong outpatient musculoskeletal assessment and treatment skills
• Skilled in therapeutic exercise prescription and manual therapy techniques
• Comfortable with patient education, discharge planning, and functional goal setting
• Familiarity with documentation best practices and electronic medical records
• Good communication and team collaboration skills
• Understanding of provider enrollment and basic insurance credentialing workflows
