Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide personalized physical therapy services to patients recovering from surgery, falls, and other conditions in their homes
Develop individualized treatment plans that help patients regain strength, mobility, and independence
Collaborate with home health and hospice teams to deliver compassionate, coordinated care
Document patient progress using the HomeCare/HomeBase EMR system
Travel within a 60-mile radius to deliver care where patients need it most

Qualifications

Current North Dakota Physical Therapy License required
BLS certification required
At least 1 year of physical therapy experience, including previous home health experience
Valid driver's license and auto insurance required
Strong communication and time-management skills for independent home visits

Knowledge & Skills

Home health physical therapy expertise
Comfort working independently in patients' homes
Familiarity with HomeCare/HomeBase EMR
Ability to manage a 5-7 patient daily caseload
Knowledge of common home health diagnoses such as post-surgical recovery and fall prevention
