Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes across Tehama County
Conduct Start of Care visits especially for orthopedic patients when nursing is not ordered
Develop personalized treatment plans for geriatric patients managing multiple conditions like CHF and COPD
Complete approximately 5 meaningful patient visits each day using your clinical expertise
Document care efficiently with provided tablet and HomeCare HomeBase EMR system
Collaborate with the intake RN and office manager to coordinate excellent patient outcomes

Qualifications

Current Physical Therapy license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than 2 years experience who have Home Health background
Valid driver's license and reliable transportation for community visits

Knowledge & Skills

Physical Therapy expertise in home health settings
Comfortable with HomeCare HomeBase EMR documentation
Strong assessment and care planning abilities for orthopedic and geriatric patients
Excellent time management for managing daily visit schedules and mileage
Compassionate communication skills for building trust with patients and families
