Roles & Responsibilities

• Evaluate patients thoroughly to create individualized therapy plans
• Deliver daily treatments focused on functional gains and safe discharge
• Manage timely discharges and coordinate care for rehab-to-home success
• Document therapy sessions and outcomes accurately and efficiently
• Collaborate with interdisciplinary team members to optimize patient recovery
• Maintain productivity expectations and meet the facility requirement (90%)
• Adhere to CRG policies and facility protocols while representing the rehab team
• Be present on-site to receive scheduled/guaranteed hours and support patients in person

Qualifications

• Licensed Physical Therapist (PT) in good standing; DPT preferred
• 1-3 years of clinical experience (prior SNF experience strongly preferred)
• Comfort working in a skilled nursing facility with a rehab-to-home population
• Ability to work on-site, follow facility policies, and meet productivity standards
• Professional presentation and adherence to dress code (black scrubs)
• Available to start within approximately 2-3 weeks of assignment acceptance

Education

• Licensed Physical Therapist (PT); Doctor of Physical Therapy (DPT) preferred

Knowledge & Skills

• Skilled in evaluations, daily treatments, and discharge planning
• Strong documentation and time-management skills to meet productivity targets
• Experience with geriatric and skilled nursing patient populations
• Ability to develop patient-centered plans that support return-to-home goals
• Excellent communication and teamwork skills within interdisciplinary rehab
