Roles & Responsibilities

• Provide skilled occupational therapy evaluations and individualized treatment plans for SNF residents
• Deliver person-centered interventions that improve function, independence, and quality of life
• Collaborate with nurses, PT/SLP, and the interdisciplinary team to coordinate care
• Accurately document interventions, progress notes, and outcomes according to facility and CRG policies
• Meet productivity expectations and contribute to team performance (therapist productivity requirement 90%)
• Maintain professionalism on-site, including following dress code and being present in the building to receive guaranteed hours
• Communicate with family members and staff to support discharge planning and continuity of care

Qualifications

• Active Occupational Therapy credentials as required by state licensure (please ensure New Mexico eligibility prior to start)
• 1-3 years of clinical OT experience (as listed in the job information)
• Experience or comfort working in a Skilled Nursing Facility (SNF) setting
• Strong documentation skills and ability to meet productivity expectations
• Ability to follow company policies and procedures (CRG P & P) and respond to regional leadership direction

Knowledge & Skills

• Skilled in OT assessment and treatment planning for geriatric and post-acute populations
• Familiarity with SNF workflows and interdisciplinary collaboration
• Proficient clinical documentation and charting practices
• Good communication, time management, and organizational skills
• Ability to meet productivity targets while delivering high-quality care
• Comfortable working on-site and integrating into an established therapy team
