Roles & Responsibilities

• Evaluate residents to develop individualized occupational therapy plans that improve daily function and independence
• Deliver hands-on treatment and adaptive techniques focused on safe mobility, self-care and cognitive strategies
• Collaborate with nurses, PT, SLP and facility staff to create cohesive care plans and track progress
• Document assessments, treatment plans and progress notes clearly and timely to support continuity of care
• Educate residents and families on techniques, home modifications and adaptive equipment to promote long-term success

Qualifications

• Current, active Occupational Therapist license in Texas or ability to obtain TX licensure promptly
• Entry-level to experienced OTRs welcome; prior experience in skilled nursing/long term care a strong plus
• Current CPR/BLS certification or willingness to obtain upon hire
• Ability to pass background check and meet facility onboarding requirements
• Flexible, team-oriented mindset and strong communication skills with residents and staff

Education

• Degree in Occupational Therapy (required)

Knowledge & Skills

• Skilled in ADL/IADL assessments and restorative care planning
• Proficient with adaptive equipment recommendations and home safety evaluations
• Knowledge of documentation standards and electronic health record entry
• Strong clinical reasoning, time management and prioritization in a busy rehab environment
• Comfort collaborating with interdisciplinary teams to drive patient-centered outcomes
