Roles & Responsibilities

• Provide compassionate direct patient care and assist with daily living activities
• Administer medications and treatments according to care plans and policy
• Monitor and accurately document vital signs, patient status, and observations
• Collaborate with RNs and the interdisciplinary team to deliver coordinated care
• Maintain infection control and safety standards to protect patients and staff
• Communicate clearly with patients and families to educate and support them
• Prepare and update accurate patient records in the facility’s documentation system

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania (required)
• Current BLS/CPR certification (required or must obtain upon hire)
• Graduate of an accredited LPN/Vocational Nursing program
• Ability to pass background check and drug screening as required by facility
• Strong communication, organization, and teamwork skills
• Prior clinical experience preferred but new graduates may be considered depending on facility needs

Education

• Graduate of an accredited LPN/LVN program or equivalent vocational nursing education

Knowledge & Skills

• Medication administration and basic IV/infusion familiarity where applicable
• Patient assessment and basic clinical evaluation skills
• Charting and documentation best practices (EHR familiarity is a plus)
• Infection control principles and safe patient handling
• Excellent interpersonal and patient-family communication skills
• Ability to follow care plans and escalate changes in condition appropriately
