Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients
Administer medications and treatments safely and accurately
Monitor patient status, document changes, and communicate concerns to the care team
Assist with activities of daily living and promote patient comfort and dignity
Collaborate with nursing and interdisciplinary staff to deliver coordinated care
Participate in shift handoffs and maintain accurate, timely charting

Qualifications

Active LPN license (practical/vocational nurse) — ability to practice in Pennsylvania preferred
Current CPR/BLS certification or ability to obtain upon hire
Professional bedside manner with strong communication and teamwork skills
Previous LPN experience in long-term care, skilled nursing, or similar setting preferred
Ability to pass required background checks and health screenings

Education

LPN diploma, vocational nursing certificate, or equivalent completion of an accredited practical/vocational nursing program

Knowledge & Skills

Medication administration and documentation
Basic assessment and monitoring of patient vital signs
Electronic health record (EHR) charting and accurate record keeping
Infection control practices and safety protocols
Time management and prioritization in a fast-paced care environment
