Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct patient care following plan of care and facility policies
Administer medications and treatments accurately and document appropriately
Monitor and record patient condition and communicate changes to the care team
Collaborate with RNs, physicians, and interdisciplinary team members to support patient outcomes
Assist with ADLs, mobility, and comfort measures while promoting patient dignity
Participate in shift handoffs and maintain clear, professional documentation

Qualifications

Current LPN license in Pennsylvania (required)
Completion of an accredited practical nursing program
Current BLS/CPR certification (or obtain upon hire)
Prior experience in a long-term care or skilled nursing setting preferred
Strong communication skills and a team-first attitude
Ability to pass background and health screenings per facility policy

Education

Graduate of an accredited Practical Nursing (LPN) program

Knowledge & Skills

Foundational nursing assessment and documentation skills
Medication administration and basic IV/PO knowledge
Familiarity with infection control and patient safety practices
Comfortable working in a fast-paced healthcare environment
Good time management and prioritization under supervision
