Roles & Responsibilities

Provide compassionate, high-quality bedside care to residents and patients
Administer medications and treatments accurately and according to protocol
Monitor and document patient conditions, reporting changes promptly
Collaborate with nurses, CNAs, and interdisciplinary team members for coordinated care
Assist with admissions, discharges, and patient education to support continuity of care
Maintain a safe, clean environment and follow infection control standards

Qualifications

Active LPN license in Pennsylvania (required)
Current CPR/BLS certification (or willingness to obtain)
Ability to pass background check and drug screen
Strong communication skills and a patient-centered mindset
Comfortable using electronic medical records and clinical documentation

Knowledge & Skills

Medication administration and basic IV/shot protocols
Vital signs assessment and patient monitoring
Wound care basics and infection control practices
Accurate clinical documentation and charting in EMR systems
Ability to prioritize tasks in a fast-paced care environment
Teamwork and effective communication with interdisciplinary staff
