Roles & Responsibilities

Assess and monitor patient conditions, making timely observations and reporting changes
Administer medications and treatments accurately and safely
Document patient care thoroughly and maintain clear clinical records
Collaborate with RNs, physicians, and care teams to deliver coordinated care
Provide compassionate patient and family education to support recovery and wellbeing
Assist with wound care, vital signs, and routine nursing procedures with attention to detail
Promote a safe, clean, and respectful care environment for residents and staff

Qualifications

Active LPN license (PA license preferred) — your core credential
Current CPR/BLS certification or willingness to obtain upon hire
Comfortable administering meds, performing treatments, and documenting care
Compassionate bedside manner and solid communication skills
Prior experience in long-term care, skilled nursing, or similar settings is a plus

Education

Diploma or certificate in Practical Nursing (LPN) or equivalent
High school diploma or GED

Knowledge & Skills

Medication administration and basic nursing procedures
Vital signs assessment, wound care, and ADL assistance
Charting and electronic medical records familiarity
Infection control practices and patient safety mindset
Strong interpersonal skills for teamwork and resident-family interactions
