Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients
Administer medications and treatments accurately and safely
Assess patient status and report changes to the care team promptly
Document care, observations, and communication clearly and professionally
Collaborate with nurses, therapists, and support staff to deliver coordinated care
Support patient comfort and dignity while promoting independence

Qualifications

Current, active New York LPN license (required)
Basic Life Support (BLS) certification preferred or willingness to obtain
Prior experience in long-term care, skilled nursing, or similar settings a plus
Strong communication skills and a patient-centered approach
Reliable, professional, and able to work collaboratively within a team

Education

Completion of an accredited LPN program or equivalent credential

Knowledge & Skills

Medication administration and documentation best practices
Basic nursing assessments and care planning
Infection control and safe patient handling
Electronic health record documentation or charting familiarity
Clear, compassionate communication with patients, families, and team members
