Roles & Responsibilities

Provide compassionate daily nursing care to residents, ensuring dignity and comfort
Administer medications and treatments accurately and document thoroughly
Monitor and record vital signs, reporting changes in condition promptly
Collaborate with nurses, therapists, and care staff to deliver coordinated care
Assist with activities of daily living and promote patient independence
Participate in care planning, handoffs, and effective patient communication
Maintain a safe, clean environment and adhere to infection control standards

Qualifications

Current LPN license in Connecticut or eligibility to obtain state licensure
CPR/BLS certification (current) or willingness to obtain upon hire
Previous experience in long-term care or skilled nursing preferred but new graduates welcome
Strong communication, teamwork, and patient-centered care skills
Ability to pass background checks and meet facility onboarding requirements

Education

Practical Nursing diploma or completion of an accredited LPN program

Knowledge & Skills

Medication administration and accurate documentation
Basic wound care, ADL assistance, and patient mobility support
Vital signs assessment and clinical observation skills
Familiarity with infection control and safety protocols
Comfort using electronic medical records and charting systems
Time management, prioritization, and teamwork in a clinical setting
