Roles & Responsibilities

Provide compassionate direct patient care consistent with LPN scope of practice
Administer medications and treatments accurately and document observations
Monitor and report changes in resident condition to the care team promptly
Collaborate with RNs, CNAs, and interdisciplinary staff to create a safe care environment
Maintain accurate, timely charting in electronic or paper medical records
Support residents' activities of daily living with respect, dignity, and professionalism

Qualifications

Current LPN licensure (Pennsylvania or compact/state-appropriate) — required
Graduation from an accredited practical nursing program
Basic Life Support (BLS/CPR) certification preferred or required per facility policy
Previous experience in long-term care or skilled nursing is a plus
Strong communication skills and the ability to work well in a team-focused environment

Education

Graduate of an accredited LPN/Vocational Nursing program
High school diploma or equivalent

Knowledge & Skills

Fundamentals of nursing care and medication administration
Charting and documentation best practices
Patient assessment and recognition of clinical changes
Infection control and safety protocols
Effective interpersonal skills with residents, families, and staff
