Roles & Responsibilities

• Provide compassionate direct patient care, following care plans and clinical guidelines
• Administer medications and treatments safely and accurately
• Monitor resident condition, document changes, and communicate concerns to the care team
• Assist with activities of daily living and promote resident dignity and independence
• Support admissions, transfers and discharges with clear, timely documentation
• Collaborate with nurses, therapists and facility staff to deliver coordinated care
• Participate in care planning and contribute observations that improve resident outcomes
• Maintain accurate and timely charting while adhering to facility policies and regulations

Qualifications

• Active LPN license in Pennsylvania (must be in good standing)
• Practical Nursing diploma or equivalent from an accredited program
• Current CPR/BLS certification or willingness to obtain upon hire
• Strong clinical assessment and medication administration skills
• Excellent communication skills and the ability to work well in a team
• Background check and any required immunizations or health screenings

Education

• Practical Nursing diploma or certificate (required)
• High school diploma or GED (required)

Knowledge & Skills

• Knowledge of long-term care best practices and resident-centered care
• Proficiency with medication administration and documentation
• Ability to prioritize tasks in a fast-paced clinical environment
• Comfortable using electronic health records and standard clinical tools
• Infection control practices and safe patient handling techniques
• Empathy, patience, and strong interpersonal skills
