Roles & Responsibilities

• Coordinate comprehensive care and discharge planning for adult and geriatric patients in an acute care setting
• Manage patient caseloads (ratios approximately 1:25) to ensure timely, safe transitions of care
• Document thoroughly in Epic and other computerized systems to maintain clear, compliant patient records
• Arrange DME, SNF and ambulance transport independently and efficiently
• Lead and participate in family meetings to develop and communicate patient-centered treatment plans
• Collaborate with interdisciplinary teams and community resources, including HMOs/IPAs and workers' compensation contacts
• Ensure adherence to regulatory standards (CMS, DRG, HIPAA, OSHA, The Joint Commission) in daily practice
• Support appeals and denials processes and contribute to quality improvement efforts

Qualifications

• California state license required (pending license accepted but must be active on start date)
• Minimum 2 years acute medical Care Manager experience performing care coordination (travel experience required — hard stop)
• Epic charting experience and strong computerized documentation skills
• Comfort floating to the Laguna facility and working within variable unit needs
• Willingness to participate in weekend rotation and meet RTO expectations (no more than 7 days off restriction)

Knowledge & Skills

• Care coordination, discharge planning, and acute care case management
• DME ordering and arranging SNF/ambulance transport
• Worker's compensation case management and appeals/denials familiarity
• Regulatory knowledge: CMS, DRG, HIPAA, OSHA, The Joint Commission
• Experience in Emergency Department, ICU, and Surgical Services preferred
• Strong family meeting facilitation and interdisciplinary collaboration skills
• Proficient charting in Epic and other electronic documentation systems
