Roles & Responsibilities

• Provide compassionate, hands-on nursing care to rehabilitation patients
• Administer medications and treatments safely and according to policy
• Perform patient assessments and communicate changes in condition
• Support rehabilitation activities and assist patients with ADLs
• Collaborate with the interdisciplinary team to create meaningful care plans
• Accurately document patient care, interventions, and responses in the chart
• Maintain infection control standards and promote a safe care environment
• Assist with admissions, transfers, and discharges to ensure smooth transitions

Qualifications

• Current LPN OR RN licensure (appropriate state licensure required)
• Active BLS/CPR certification (or willingness to obtain promptly)
• Clinical experience in rehabilitation, long-term care, or similar setting preferred
• Ability to work day shift hours (6:30 AM - 7:00 PM) and follow facility scheduling
• Strong communication skills and a patient-centered approach

Education

• Graduate of an accredited LPN program or RN diploma/associate/BSN program

Knowledge & Skills

• Skilled in medication administration and documentation
• Proficient with patient assessments, vital signs, and care planning
• Familiarity with rehab nursing principles and supporting ADLs
• Good clinical judgment, time management, and teamwork
• Comfortable using electronic medical records and following facility policies
