Roles & Responsibilities

• Assess speech, language, cognitive-communication, and swallowing function for geriatric patients
• Develop individualized treatment plans that prioritize patient-centered goals and measurable progress
• Deliver skilled therapy interventions across the continuum of care in a geriatric rehab setting
• Collaborate closely with nursing, PT/OT, physicians, and family members to coordinate care
• Document evaluations, progress notes, and discharge summaries in a timely, compliant manner
• Provide caregiver and family training to support carryover and safe discharge planning
• Participate in interdisciplinary team meetings and contribute clinical expertise to patient plans
• Support quality initiatives and maintain caseload continuity when splitting coverage with partner sites

Qualifications

• State licensure in Speech-Language Pathology or eligibility to obtain licensure in Georgia
• ASHA CCC-SLP preferred or the ability to obtain certification
• Experience working with geriatric or post-acute populations preferred
• Demonstrated ability to perform instrumental and clinical swallowing assessments
• Strong communication and interdisciplinary collaboration skills
• Current background check and ability to meet facility onboarding requirements

Education

• Master's degree in Speech-Language Pathology (or related graduate degree required)

Knowledge & Skills

• Skilled in assessment and treatment of dysphagia and neurogenic communication disorders
• Proficient documentation and electronic medical record usage
• Knowledge of geriatric rehabilitation principles and discharge planning
• Ability to educate caregivers and other clinicians in functional communication strategies
• Time management and caseload prioritization in a busy rehab setting
