Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents consistent with LPN scope
• Administer medications and treatments accurately and document thoroughly
• Monitor vital signs and promptly report changes in patient condition
• Collaborate with RNs, physicians, and the care team to develop and follow care plans
• Perform wound care, ADL assistance, and skilled nursing procedures as directed
• Educate patients and families, offering clear instructions and emotional support
• Maintain accurate charts and electronic health records with timely documentation

Qualifications

• Active LPN license in Virginia or ability to obtain prior to start
• Current BLS/CPR certification required
• Up-to-date immunizations and ability to pass background check and drug screen
• Previous experience in long-term care, skilled nursing, or similar setting preferred
• Strong communication skills and a team-oriented mindset

Education

• Diploma or certificate in Practical Nursing (LPN) from an accredited program

Knowledge & Skills

• Proficiency with medication administration and basic nursing procedures
• Comfortable with electronic health record charting and documentation
• Skilled at clinical assessments, vital sign interpretation, and reporting
• Good time management and prioritization in busy clinical environments
• Compassionate patient-centered care and effective family communication
