Roles & Responsibilities

• Provide compassionate, patient-centered nursing care consistent with LPN scope of practice
• Administer medications and treatments accurately and document thoroughly
• Monitor and report changes in patient condition to the care team promptly
• Assist with ADLs, mobility, and comfort measures while promoting patient dignity
• Collaborate with RNs, physicians, and interdisciplinary staff to support care plans
• Maintain accurate, timely charting in the facility electronic health record

Qualifications

• Active Licensed Practical Nurse (LPN) license — must be valid for practice
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Clinical experience in a similar care setting preferred
• Excellent communication skills and a team-oriented attitude
• Ability to pass facility/background checks and meet onboarding requirements

Education

• Practical nursing diploma or equivalent vocational training

Knowledge & Skills

• Medication administration and documentation best practices
• Basic patient assessment and vital sign monitoring
• Familiarity with electronic health record (EHR) charting
• Infection control and safety protocols
• Strong prioritization and time management on busy shifts
