Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 4:00 PM; plus 2-3 weekend days (Day shift)
Hours per Week: 37
Candidate Pay Rate: $67 per hour

Roles & Responsibilities
- Manage and optimize patient visit schedules to balance productivity and patient needs
- Perform comprehensive OASIS and physical therapy assessments, documenting psychosocial, safety and risk factors
- Develop individualized plans of care with measurable goals and logical, progressive interventions
- Deliver skilled physical therapy treatments using evidence-based therapeutic techniques
- Provide patient and caregiver education to maximize functional independence and safety
- Coordinate care with physicians, nurses, caregivers and other rehab team members as the patient’s care manager
- Supervise and guide PTAs, PT students and home care aides in clinical activities and documentation
- Complete timely clinical, billing and Medicare documentation (including OASIS) in accordance with agency, state and federal guidelines
- Use McKesson workflows to lock visits, complete follow-up notes and ensure accurate visit completion
- Participate in in-service trainings, quality improvement activities and peer review audits
- Engage in community outreach and marketing to promote rehab services and specialty programs
- Perform patient-related assessments and procedures such as vitals, wound care and staple/suture removal
Qualifications
- Licensed Physical Therapist (PT) in Vermont or eligible for Vermont licensure
- Demonstrated competency with OASIS and Medicare documentation practices
- Experience with McKesson or similar home health EMR systems is highly desirable
- Comfort supervising PTAs, students and home care aides and providing constructive feedback
- Strong care coordination, communication and documentation skills
- Experience in home health or community-based rehab settings preferred
- Current CPR/BLS certification (or willingness to obtain)
Education
- Doctor of Physical Therapy (DPT) or Master's in Physical Therapy preferred
- Relevant professional licensure required
Knowledge & Skills
- Skilled in patient assessment, development of plan of care and therapeutic techniques
- Proficient in clinical documentation, Medicare documentation and billing support processes
- Knowledgeable about APTA guidelines and standards of care
- Ability to perform wound care, staple/suture removal and routine clinical procedures
- Experience with care coordination, interdisciplinary communication and discharge planning
- Familiarity with quality improvement processes and in-service training delivery
- Comfort using McKesson workflows and other electronic health record systems
