Roles & Responsibilities

• Perform thorough patient assessments to inform care plans and ensure resident safety
• Administer medications accurately and document per policy
• Provide compassionate ADL assistance to support resident dignity and comfort
• Deliver wound care and wound VAC management with attention to detail
• Manage gastrostomy/enteral tube care and urinary catheter care safely
• Administer nebulized treatments and support CPAP care for respiratory needs
• Coordinate admissions and discharges efficiently and professionally
• Notify MD/NP of pertinent changes in resident condition and follow through on treatment orders
• Carry out assigned treatments and interventions while maintaining clear documentation

Qualifications

• Active Licensed Practical Nurse (LPN) credential
• Experience working in a Long Term Care (LTC) setting
• Ability to perform patient assessments, medication administration, ADL assistance, and wound care
• Experience with gastrostomy/enteral tube care, urinary catheter care, nebulized treatments, wound VAC and CPAP care
• Proof of flu vaccination or a facility-approved medical or religious exemption prior to contract start
• Willingness to work varied/rotating shifts based on facility need

Education

• Licensed Practical Nurse (LPN) — active licensure required

Knowledge & Skills

• Skilled in patient assessments and clinical observation
• Medication administration and accurate documentation
• ADL assistance and supportive resident care
• Wound care and wound VAC familiarity
• Gastrostomy/enteral tube and urinary catheter management
• Nebulized treatment administration and CPAP support

• Clear communication with MDs/NPs and interdisciplinary teams
