Shift Type: Rotating two-week schedule: Week 1: Saturday & Sunday 1:00 PM - 10:30 PM; Wednesday 1:00 PM - 10:30 PM; Thursday 9:00 PM - 6:30 AM. Week 2: Saturday & Sunday 6:00 AM - 10:30 PM; Wednesday 2:00 PM - 10:30 PM; Friday 10:00 PM - 6:30 AM. Includes both PM and NOC shifts, rotating schedule, every weekend.

Roles & Responsibilities

• Communicate promptly with physicians, families, and the care team about significant resident changes
• Complete comprehensive resident assessments and document findings accurately
• Administer medications following the 5 Rights with meticulous attention
• Provide wound care and perform regular wound assessments as needed
• Support CNAs by assisting with call lights and coordinating resident needs
• Monitor residents for changes in condition and act decisively to ensure safety
• Lead on the floor when needed by assessing situations and directing care
• Report any allegations of abuse, neglect, or misappropriation to leadership
• Maintain accurate and thorough resident records with timely charting

Qualifications

• Current Wisconsin Registered Nurse license required
• Associate Degree in Nursing required; BSN preferred
• Current CPR/BLS certification required
• 1+ years of related nursing experience preferred, long-term care experience a plus
• Exceptional attention to detail and strong clinical assessment skills
• Comfortable leading on the floor and collaborating with CNAs, families, and physicians

Education

• Associate Degree in Nursing (required)
• Bachelor of Science in Nursing (BSN) preferred

Knowledge & Skills

• Medication administration and safe documentation practices
• Wound care techniques and assessment
• Resident assessment and ongoing monitoring
• Accurate charting and electronic medical record familiarity
• Strong communication and teamwork with interdisciplinary staff
• Ability to prioritize and lead during busy shifts
