Roles & Responsibilities

• Provide compassionate, hands-on care to residents including bathing, grooming and assistance with activities of daily living
• Take and record vital signs accurately and report changes to the nursing team
• Assist with safe transfers and mobility support to promote independence
• Document care thoroughly and timely in the facility charting system
• Follow infection control and safety protocols to protect residents and staff
• Collaborate with nurses and interdisciplinary team members to support resident care plans
• Support mealtime assistance and feeding as needed while encouraging resident dignity
• Participate in shift handoffs and communicate changes in resident condition clearly

Qualifications

• Current and valid LNA (Licensed Nursing Assistant) certification for VT/NH as required
• CPR/BLS certification preferred or ability to obtain prior to start
• Must have reliable transportation to and from the facility
• Ability to work evenings and commit to every other weekend coverage
• Clear background check and required immunizations/documentation per facility policy
• Strong communication skills and a team-oriented attitude

Education

• Completion of an accredited CNA/LNA program or equivalent
• High school diploma or GED preferred

Knowledge & Skills

• Hands-on experience with ADLs and basic resident care
• Comfort with vital signs monitoring and basic clinical observations
• Knowledge of infection prevention and safe patient handling
• Good documentation and charting practices
• Professional communication with residents, families and clinical staff
• Time management and ability to prioritize tasks during busy shifts
