Roles & Responsibilities

• Provide skilled speech-language pathology services to SNF residents, delivering evaluations and individualized treatment plans
• Assess and manage dysphagia and swallowing disorders with evidence-based approaches
• Document progress and treatment outcomes clearly and timely in the facility’s medical record
• Collaborate with nursing, therapy colleagues, and case management to support safe discharges and continuity of care
• Educate residents and families on therapy goals, strategies, and home carryover to maximize outcomes
• Support FMLA-related documentation and processes as required by the facility
• Adapt treatment plans to meet the needs of a diverse geriatric population in a skilled nursing environment

Qualifications

• ASHA Certificate of Clinical Competence (CCC-SLP) required
• Active state SLP license (able to practice in Tennessee) required
• 1-3 years of experience working as an SLP, preferably with SNF/geriatric populations
• Experience with FMLA documentation and procedures preferred
• Comfort with some floating between units as needed
• Current professional liability coverage and ability to pass background and health screenings

Knowledge & Skills

• Skilled in bedside swallowing evaluations and dysphagia management
• Strong clinical assessment and treatment planning abilities for cognitive-communication disorders
• Proficient documentation practices and comfort with electronic medical records
• Able to collaborate effectively with interdisciplinary teams
• Good communication skills for resident/family education and care coordination
• Knowledge of SNF clinical workflows and regulatory requirements
