Roles & Responsibilities
- Provide patient-centered occupational therapy evaluations and interventions in a skilled nursing facility
- Develop and implement individualized treatment plans focused on functional independence and safe discharge
- Document therapy sessions and progress notes thoroughly and timely using facility-required systems
- Collaborate with nursing, PT, SLP and interdisciplinary team members to coordinate care
- Train patients and caregivers on adaptive equipment, energy conservation, and home safety
- Monitor outcomes and adjust plans to ensure measurable progress toward goals
- Support a positive, rehabilitative culture that inspires patients to meet milestones
Qualifications
- Licensed Occupational Therapist in Ohio or eligible for Ohio licensure
- Graduation from an accredited Occupational Therapy program (OTD/MS/BS entry-level)
- New graduates are welcome — strong mentorship and support provided
- Clean background check and up-to-date immunizations as required by facility
- Strong documentation skills and ability to manage caseload in an SNF setting
- Reliable, professional communication and teamwork skills
Education
- Completion of an accredited Occupational Therapy program (OTD or Master's recommended)
Knowledge & Skills
- Skilled in OT evaluations, ADL retraining, mobility and fall prevention
- Comfortable with therapy documentation and clinical outcome tracking
- Knowledge of SNF/long-term care environment and Medicare/Medicaid basics
- Experience with adaptive equipment, splinting basics, and caregiver training
- Ability to prioritize caseload, manage time, and collaborate with interdisciplinary teams
