Roles & Responsibilities
- Deliver skilled, patient-centered physical therapy in home settings to adults and geriatrics
- Perform thorough assessments and create individualized OASIS-compliant plans of care
- Document accurately and efficiently using EPIC Remote and Rover (OASIS experience preferred)
- Provide safe lifting, transfer, gait training and adaptive-device instruction to improve mobility
- Educate patients and families on home exercise programs, safety and fall prevention
- Coordinate care and appointments, communicate clearly with interdisciplinary teams
- Maintain productivity standards (may visit up to ~6 patients/day; 30 productivity points standard)
- Practice infection control and professional standards while traveling between client homes
Qualifications
- Licensed Physical Therapist — OR and WA licensure required (pending licenses accepted; confirm with hiring manager)
- Minimum 1 year of skilled home health physical therapy experience preferred (first-timers accepted)
- BCLS (Basic Cardiac Life Support) certification required
- Reliable transportation and willingness to drive up to ~60 miles/day (occasional longer drives)
- Comfortable using EPIC Remote, Rover and completing OASIS documentation (OASIS experience preferred)
- Strong communication, time management and patient education skills
Education
- Doctor of Physical Therapy (DPT) preferred or Bachelor's in Physical Therapy (or equivalent)
- State PT licensure (OR and WA) required
Knowledge & Skills
- Home Health best practices and OASIS documentation
- Adaptive devices, body mechanics, lifting & transfer techniques
- Pain assessment, skin integrity checks and wound-care awareness
- Strength testing, vital sign monitoring and mobility/gait training
- Orthopedics, neurology, geriatrics and oncology rehabilitation basics
- Use of Hoyer lifts, gait belts, walkers, wheelchairs and prosthetic considerations
- Modalities familiarity (E-stim, TENS, ultrasound, cryotherapy) and cardiopulmonary techniques
- Epic Remote charting, Rover app workflow, efficient documentation and reassessments
