Roles & Responsibilities
- Provide patient-centered physical therapy evaluation, treatment, and reassessment in home settings
- Document OASIS-compliant assessments and plan-of-care entries using Epic and Rover
- Perform safe lifting, transfer training, and patient transport to/from treatment
- Teach patients and families home exercise programs and strategies for safety and independence
- Assess skin integrity, pain, strength, vital signs and modify treatment plans accordingly
- Coordinate care via telephone calls and scheduling to ensure seamless visits
- Collaborate with interdisciplinary teams to support discharge planning and continuity of care
Qualifications
- Active OR and WA Physical Therapist licensure required (confirm licensure status with recruiter; pending may be discussed)
- Minimum 1 year of skilled home health physical therapy experience preferred
- BCLS (Basic Cardiac Life Support) required
- Reliable transportation and willingness to travel within a ~60 mile radius (occasional longer days possible)
- Experience with OASIS documentation preferred — please note if you do not have OASIS experience
- Exceptional time management, charting accuracy, and strong patient education skills
Education
- Doctor of Physical Therapy (DPT) or equivalent entry-level PT degree (state-required credential)
- Completion of state licensure requirements for OR and WA
Knowledge & Skills
- Home Health clinical skills: adaptive devices, AROM/AAROM/PROM, body mechanics, strength testing
- Assessment and documentation: OASIS, plan of care, reassessments, discharge planning
- Patient safety: lifting and transfer techniques, gait training, wheelchair assessment
- Modalities familiarity: cryotherapy, e-stim, moist heat, TENS, ultrasound
- Experience with cardiopulmonary, neurologic, orthopedic, and geriatric populations
- Comfortable using Epic (remote) and Rover mobile app for charting
- Strong communication: patient/family education and care coordination via telephone
