Roles & Responsibilities
- Provide skilled home health physical therapy visits focused on safe mobility, strength, transfers and functional outcomes
- Conduct thorough assessments and document OASIS-compliant evaluations and plans of care in EPIC/Rover
- Deliver individualized treatment including adaptive device training, gait and balance training, and wound/skin integrity management
- Educate patients and families on home exercise programs, safety strategies and falls prevention
- Collaborate with interdisciplinary team and coordinate care, supplies and follow-up appointments
- Manage daily schedule efficiently to meet productivity standards (standard: 30 points; up to 6 visits/day possible)
- Maintain excellent time management and travel planning to minimize windshield time while maximizing patient care
Qualifications
- Active OR and WA physical therapy licensure required (must have BOTH)
- BCLS (basic cardiopulmonary life support) certification required
- Minimum 1 year of skilled home health experience required (first-time travel clinicians considered)
- OASIS documentation experience required — please call out if you do not have OASIS experience
- Reliable transportation and valid driver’s license for daily travel between patient homes
- Comfortable using EPIC Remote/Rover for charting and mobile documentation
- Preferential: Wound care, Hand Therapy, Manual Therapy certifications or experience
Education
- Doctor of Physical Therapy (DPT) or Master’s degree in Physical Therapy; active state licensure in OR and WA
Knowledge & Skills
- Adaptive devices, assistive device training and mobility assessments
- Body mechanics, safe lifting and transfer techniques
- Pain assessment, skin integrity monitoring and wound care basics
- Strength testing and vital sign monitoring with clinical decision-making
- Gait training, prosthetic management and functional mobility interventions
- Cardiopulmonary and neurologic rehab knowledge (COPD, CHF, post-stroke, Parkinsons, dementia)
- Modalities familiarity (cryotherapy, E-stim, TENS, ultrasound) and therapeutic exercise progression
- Excellent documentation skills for OASIS, plan of care, reassessments and discharge
