Roles & Responsibilities

Provide compassionate care coordination and support to patients and families
Conduct thorough psychosocial assessments and develop individualized care plans
Collaborate with interdisciplinary teams to ensure seamless patient care
Facilitate 5150 evaluations and involuntary holds when clinically appropriate
Connect patients with vital community resources and support services

Qualifications

Active California LCSW license required
Current BLS certification
5150 certification in California required
Prior Dignity Health experience strongly preferred
Strong assessment and crisis intervention skills

Knowledge & Skills

Expertise in involuntary psychiatric holds and 5150 processes
Knowledge of local community resources and support networks
Ability to work collaboratively within multidisciplinary teams
Strong documentation and communication abilities
