Candidate Pay Rate: $49 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes across greater Sacramento county
Help patients recover from cardio, pulmonary, orthopedic, neurological, and geriatric conditions
Develop personalized treatment plans that empower patients to regain independence
Collaborate with interdisciplinary teams to deliver exceptional home health care
Document patient progress and communicate effectively with supervisors and care teams

Qualifications

Active Physical Therapy Assistant license in California
Minimum experience requirements apply (new grads considered only in exceptional cases)
Strong communication and interpersonal skills
Ability to work independently in home health settings
Valid driver's license and reliable transportation

Knowledge & Skills

Home health care experience preferred
Knowledge of various diagnoses including cardio, pulmonary, orthopedic, neurological, and geriatric conditions
Ability to manage a caseload of 5-6 patients per day
Strong documentation and time management skills
Comfortable traveling between patient homes in the greater Sacramento area
