Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating weekend shifts 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Provide start of care, ROC, recertifications, evaluations, and daily visits for home health patients
Deliver physical therapy services to patients with CHF, orthopedic conditions, and pneumonia
Cover assigned home health areas throughout Louisville
Collaborate with clinical managers and educators for patient care support

Qualifications

BLS certification required
Physical Therapist license
Home health experience preferred
No new grads preferred

Knowledge & Skills

Home health physical therapy expertise
Patient assessment and evaluation skills
Care planning and documentation
Working with CHF, orthopedic, and pneumonia patients
