Roles & Responsibilities

Deliver personalized physical therapy services in patients' homes across Yolo County
Assess and evaluate each patient's functional status to create tailored care plans
Guide patients and families through therapeutic exercises and safe use of equipment
Collaborate with physicians and care teams to adjust treatment based on patient progress
Document clinical notes and progress summaries that support quality patient outcomes
Empower patients to regain independence and improve their quality of life

Qualifications

Minimum 2 years of physical therapy experience in acute, home care, or rehab settings
Current BLS certification required
Strong communication and organizational skills
Valid driver's license and reliable transportation for home visits

Knowledge & Skills

Home health physical therapy assessment and treatment techniques
Patient and caregiver education on therapeutic appliances and exercises
Understanding of Medicare, Medi-Cal, and Title 22 regulations
Ability to work independently while collaborating with interdisciplinary teams
