Roles & Responsibilities

• Provide compassionate hospice nursing care to terminally ill patients across a variety of diagnoses
• Serve as case manager for a caseload of approximately 8-10 patients weekly, ensuring continuity and quality of care
• Perform biweekly supervisory visits of home health aides and coordinate care delivery
• Manage durable medical equipment (DME) needs and ensure proper use and documentation
• Document patient encounters and care plans thoroughly in EPIC to maintain accurate clinical records
• Participate in the rotating weekend and holiday on-call schedule and respond professionally to urgent needs
• Collaborate with interdisciplinary teams, families and caregivers to develop patient-centered care plans
• Support and mentor home health/hospice aides and contribute to a positive unit culture

Qualifications

• Active Registered Nurse (RN) license
• BLS (AHA) certification required
• National Hospice Certification preferred
• Minimum 1 year combined med-surg and hospice experience
• Driver's license required (60 mile radius; driver’s license must be submitted)
• Comfortable with on-call duties and biweekly supervisory travel visits
• Ability to be in-state for the week of orientation even if some modules are remote

Education

• Active RN licensure (ADN/BSN or diploma acceptable); BSN not required

Knowledge & Skills

• Strong hospice and palliative care knowledge and compassionate patient-centered approach
• Case management skills including care coordination and utilization review
• Proficiency with EPIC charting and clinical documentation
• Experience with DME management and coordinating equipment needs
• Solid med-surg clinical foundations
• Effective communication, clinical leadership and supervisory skills
