Candidate Pay Rate: $49 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes across greater Sacramento county
Treat a diverse caseload including cardio, pulmonary, orthopedic, neurological, and geriatric patients
Help patients regain mobility and independence with personalized treatment plans
Collaborate with interdisciplinary teams to deliver exceptional home health care
Document patient progress and communicate effectively with supervisors and care teams
See 5-6 patients per day while maintaining high-quality care standards

Qualifications

Active Physical Therapy Assistant license in California
Minimum experience requirements apply - new grads considered only in exceptional cases
Strong clinical skills across multiple specialties including cardio, pulmonary, orthopedic, neurological, and geriatric care
Reliable transportation for home visits throughout the service area
Excellent communication and documentation abilities

Knowledge & Skills

Home health care experience preferred
Ability to work independently in various home settings
Strong organizational and time management skills
Compassionate bedside manner with diverse patient populations
Proficiency with electronic documentation systems
