Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes
Conduct start of care assessments and resumption of care visits
Complete recertifications and daily treatment visits with a manageable caseload
Collaborate with clinical coordinators and fellow therapists for seamless patient care
Perform safety evaluations and teaching visits in hospice situations when needed
Document patient progress using OASIS and maintain accurate clinical records

Qualifications

Current ND Physical Therapy License required
BLS certification required
Valid Driver's License and Auto Insurance
Home Health experience preferred
OASIS training preferred
Open to first-time travelers

Knowledge & Skills

Physical Therapy expertise in home health settings
OASIS documentation proficiency
Strong communication and patient education skills
Ability to work independently in the field
Comfortable with a 6.5-point daily productivity standard
