Roles & Responsibilities
- Provide compassionate care coordination for patients in need of psychiatric evaluation
- Complete 5150 involuntary holds when individuals are believed to be a danger to themselves or others
- Collaborate with interdisciplinary teams to ensure seamless patient support
- Maintain accurate documentation and follow all regulatory guidelines
Qualifications
- Active California LCSW license required
- Current BLS certification
- 5150 certification in California required
- Prior Dignity Health experience in California mandatory
- Two supervisory references needed at time of submittal
Knowledge & Skills
- Expertise in conducting 72-hour psychiatric holds
- Strong assessment and crisis intervention skills
- Ability to work collaboratively within care coordination teams
- Knowledge of California mental health regulations
