Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to 5-7 patients daily in their homes
Help patients recover from surgeries, falls, and other conditions with personalized care plans
Travel within a 60-mile radius around Valley City to deliver exceptional home health support
Document patient progress using the HomeCare/HomeBase EMR system
Collaborate with interdisciplinary teams to ensure seamless patient outcomes
Educate patients and families on exercises and recovery techniques for lasting results

Qualifications

Current North Dakota Physical Therapy License required
BLS certification needed to ensure patient safety
At least 1 year of previous home health experience preferred
Valid driver's license and auto insurance essential for travel between patient homes
Strong PT-related clinical skills to support diverse patient needs

Knowledge & Skills

Home health physical therapy expertise
Comfortable working independently in patients' homes
Familiarity with HomeCare/HomeBase EMR documentation
Excellent communication and patient education abilities
Ability to manage a caseload of 5-7 patients per day efficiently
