Physical Therapist – UHC Staffing

Physical Therapist

Location: Walworth, WI

Contract Type: Perm

Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (Day shift, 35 hours/week, No weekend rotation)

Hours per Week: 35

Candidate Pay Rate: $40 per hour

Roles & Responsibilities

Provide patient-centered physical therapy evaluations and interventions
Develop and implement individualized treatment plans to restore function and mobility
Monitor and document patient progress clearly and timely
Collaborate with nurses, physicians and rehab team members to optimize outcomes
Educate patients and families on home programs and safe activity progression
Participate in discharge planning to ensure continuity of care

Qualifications

Licensed Physical Therapist in the state of Wisconsin (or eligible to obtain licensure)
Current CPR/BLS certification or willingness to obtain upon hire
Strong communication skills and a collaborative mindset
Experience in inpatient rehabilitation or related settings preferred
Ability to complete required background checks and onboarding documentation

Education

Doctor of Physical Therapy (DPT) preferred; Master’s or Bachelor’s in Physical Therapy where applicable

Knowledge & Skills

Skilled in assessment and development of therapeutic treatment plans
Proficient with therapeutic exercise, gait training and functional mobility interventions
Solid documentation practices and attention to clinical detail
Ability to work effectively as part of an interdisciplinary team
Patient education and family coaching for home exercise adherence
