Roles & Responsibilities

• Perform thorough evaluations to assess patients' physical abilities and craft individualized treatment plans
• Provide hands-on physical therapy interventions to improve strength, mobility, and daily function
• Educate patients and families on therapeutic exercises, safe movement techniques, and home modifications
• Collaborate with occupational therapists, nurses, and physicians to coordinate care and optimize outcomes
• Monitor patient progress and adapt treatment plans to achieve measurable improvement
• Maintain accurate, timely clinical documentation in accordance with facility policies and regulations
• Ensure care plans align with best practices and regulatory requirements

Qualifications

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
• Valid Physical Therapy license in California
• Experience in a skilled nursing facility (SNF) or similar healthcare setting preferred
• Strong clinical knowledge of therapeutic modalities, adaptive equipment, and therapeutic exercise
• Excellent communication, interpersonal, and problem-solving skills
• Ability to work independently while contributing to an interdisciplinary team
• Compassionate, patient-focused approach with solid documentation skills

Education

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program

• Designing and implementing individualized treatment plans
• Therapeutic exercise and mobility improvement strategies
• Use of therapeutic modalities and adaptive equipment
