Roles & Responsibilities

• Assess patients across the lifespan and develop individualized outpatient physical therapy treatment plans in consultation with physicians
• Deliver hands-on care: manual therapy, therapeutic exercise, gait and assistive device training, and modality-based treatments like hydrotherapy and ultrasound
• Monitor and evaluate patient progress, adjusting treatments and recommending prosthetic/orthotic fit changes as needed
• Create meaningful home exercise programs and provide clear education to patients, families, and caregivers for continuity of care
• Complete accurate, timely documentation and medical charting to ensure continuity and quality of care
• Lead and participate in discharge planning and care coordination with physicians, nurses, social workers, and other team members
• Maintain safe, clean treatment areas and follow infection-control protocols to protect patients and staff
• Perform preventive maintenance and basic troubleshooting on therapy equipment and escalate repairs appropriately
• Contribute to staff development by delivering in-service training and sharing best practices

Qualifications

• Possession of a Physical Therapy License issued by the State of Colorado (required)
• BLS certification (required)
• Proven ability to assess, plan, and implement outpatient physical therapy interventions for pediatric to geriatric patients
• Skilled in manual therapy techniques, therapeutic exercise prescription, and assistive device training
• Strong documentation and charting skills with attention to detail and regulatory compliance
• Comfortable administering modalities such as hydrotherapy, ultrasound, traction, and related equipment
• Customer-focused communication skills to educate patients and collaborate with interdisciplinary teams
• Willingness to participate in staff training and quality improvement initiatives

Education

• Physical Therapy License (Colorado) - required
• BLS certification - required
