Roles & Responsibilities

• Conduct thorough psycho-social and mental health evaluations to guide care
• Develop and document individualized treatment plans that address patient needs
• Provide crisis counseling and timely interventions for patients in acute need
• Coordinate discharge planning and connect patients with appropriate home and community services
• Advocate for patients with state and federal agencies to secure resources and placements
• Collaborate in interdisciplinary team meetings and contribute to patient care conferences
• Maintain timely, regulatory-compliant clinical documentation in the electronic health record
• Provide therapeutic support, education, and coaching to patients and families

Qualifications

• Must hold Alaska LCSW at time of submittal; LMSW considered when appropriate
• Minimum 2-4 years of social work experience in inpatient or outpatient healthcare settings
• NABCS Clearance and ICPA Clearance required
• Flu vaccination required; two fingerprint cards required upon contract acceptance
• At least 21 years of age
• Proven experience with mental health evaluations, treatment planning, and crisis counseling
• Competent use of electronic health records and clinical documentation practices

Education

• Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW)

Knowledge & Skills

• Strong case management and discharge planning skills
• Excellent oral and written communication, including empathic listening
• Familiarity with community resources and ability to link patients to services
• Ability to maintain confidentiality and meet regulatory documentation standards
• Comfortable using computers, EHR systems, Google Drive, fax, and phone systems
• Skilled at working collaboratively within multidisciplinary teams
• Effective conflict resolution, time management, and independent decision-making
