Roles & Responsibilities

• Assess and treat patients with movement dysfunction or pain to improve daily function, mobility, and independence
• Develop and implement individualized treatment plans focused on pain management, fall risk reduction, and mobility improvement
• Lead discharge planning efforts to ensure safe transitions and optimal functional outcomes
• Document evaluations, progress notes, daily notes, discharges, and all patient-related communications accurately and timely
• Coordinate with providers, insurance companies, patients, rehab staff, and supervisors to advocate for patient needs and secure approvals
• Support clinic upkeep by assisting with treatment room cleaning, laundry, and maintaining a safe therapeutic environment
• Perform other duties as requested to support patient care and team operations

Qualifications

• Valid Physical Therapist license in Wisconsin or eligibility to obtain state licensure
• Proven ability to evaluate patients and create effective treatment plans focused on mobility, pain, and fall prevention
• Strong documentation skills, including evaluations, progress notes, discharge summaries, and insurance communications
• Experience communicating with providers and insurance representatives regarding approvals and denials
• Comfort performing light facility duties such as treatment room cleaning and laundry to support patient care
• Excellent interpersonal and communication skills with patients, families, and interdisciplinary teams
• Current CPR/BLS certification preferred or willing to obtain

Education

• Degree in Physical Therapy (DPT preferred) or equivalent professional degree

• Physical therapy assessment and treatment planning
• Pain management techniques and fall risk reduction strategies
• Mobility improvement and functional rehabilitation
• Discharge planning and care coordination
• Clinical documentation and progress note best practices
• Experience with insurance approvals/denials and provider communication
