Roles & Responsibilities

• Provide skilled physical therapy services in patients' homes with warmth and professionalism
• Assess mobility, function, and home safety to create individualized treatment plans
• Develop and implement therapeutic interventions to improve strength, balance, and independence
• Educate patients and caregivers on safe transfers, exercise programs, and fall prevention
• Monitor progress, adjust plans of care, and document clinical outcomes clearly and promptly
• Collaborate with nurses, case managers, and family members to coordinate holistic care
• Advocate for patient needs and participate in discharge planning when appropriate
• Use clinical judgment to ensure safety, dignity, and measurable functional gains

Qualifications

• Current, unrestricted state license to practice as a Physical Therapist
• Active CPR/BLS certification (or willingness to obtain upon hire)
• Experience in home health settings preferred and a passion for community-based care
• Strong clinical assessment and manual therapy skills
• Excellent communication and patient education abilities
• Ability to pass standard background checks and required screenings
• Valid driver’s license and reliable transportation for travel between patient homes

Education

• Doctor of Physical Therapy (DPT) preferred; related master’s or bachelor’s degree considered where applicable
• Completion of accredited Physical Therapy program required

Knowledge & Skills

• Skilled in gait training, therapeutic exercise, balance and fall prevention strategies
• Proficient with functional outcome measures and home safety assessments
• Comfortable with assistive devices and adaptive equipment recommendations
• Familiarity with electronic medical records (EMR) and timely documentation practices
• Strong time management, prioritization, and independent clinical decision-making
• Compassionate patient education and caregiver coaching techniques
