Roles & Responsibilities

• Provide high-quality home health physical therapy assessments and develop individualized treatment plans
• Deliver therapeutic interventions focused on mobility, balance, strength, and functional independence
• Educate patients and caregivers on safe transfers, fall prevention, and home exercise programs
• Document patient encounters accurately and timely (Epic preferred but not required)
• Collaborate with interdisciplinary team members to coordinate patient-centered care
• Perform outcome tracking and progress updates to support care continuity and discharge planning
• Complete specialty competency testing and skills checklists to maintain clinical excellence

Qualifications

• Active Physical Therapy license in the state of practice (Iowa) — license verification required
• Basic Life Support (BLS) certification (required)
• Home health experience preferred; comfort performing independent patient visits in the home setting
• Epic experience preferred but not required
• Completion of Trinity Specialty Specific Competency Testing and Trinity Skills Checklist (per employer process)
• Current immunizations and health screening per employer requirements (MMR, Hep B, Varicella, Tdap, COVID-19, Influenza, fit test, drug screen, TB testing)

Education

• Doctor of Physical Therapy (DPT) preferred or degree as required by state licensure

Knowledge & Skills

• Strong patient assessment and clinical reasoning skills for adult and geriatric populations
• Skilled in gait training, transfers, balance/fall prevention, and therapeutic exercise
• Comfortable with home safety evaluations and adaptive equipment recommendations
• Excellent verbal and written communication; timely and accurate documentation
• Ability to work independently, manage a caseload, and collaborate with remote teams
• Familiarity with electronic medical records (Epic a plus)
