Roles & Responsibilities

• Evaluate residents and develop meaningful, resident-centered therapeutic plans of care
• Deliver therapeutic interventions and modalities to maximize functional independence
• Document all evaluative and therapeutic interactions clearly and promptly in the EMR
• Collaborate with physicians, nursing, and the interdisciplinary team on care and discharge planning
• Recommend, fit, and educate residents and caregivers on appropriate assistive devices
• Supervise and mentor assistant-level staff, ensuring safe, compliant delegation
• Provide in-service education and contribute to the development of new rehabilitation programs
• Maintain department cleanliness and follow infection control and safety procedures

Qualifications

• Active Physical Therapist license in the State of work (required)
• Bachelor's degree required; Master’s or Doctorate preferred
• Basic Life Support (BLS) certification preferred
• Prior experience in long-term care or skilled nursing facility preferred
• Strong documentation skills and comfort using Electronic Medical Records
• Ability to lift up to 50 pounds and perform the physical demands of the role
• Excellent verbal and written communication and teamwork skills

Education

• Bachelor's degree required (Master's or Doctorate preferred)

Knowledge & Skills

• Patient evaluation and therapeutic plan of care development
• Skilled use of therapeutic modalities and assistive device recommendations
• Thorough knowledge of Medicare, Medicaid, and reimbursement documentation requirements
• Proficient documentation in EMR systems and strong time management
• Interdisciplinary team communication and resident-centered care approach
• Infection control practices and clinical education delivery
• Comfortable with Microsoft Office for reporting and education materials
