Roles & Responsibilities

• Evaluate patients in the skilled nursing setting and develop individualized OT treatment plans
• Provide hands-on therapy and functional training to help patients regain independence
• Collaborate with the therapy team, nursing staff, and families to coordinate care
• Document skilled services thoroughly and timely to meet facility standards
• Meet productivity expectations and contribute to a high-performing therapy program
• Follow CRG policies and procedures to ensure consistent, quality care
• Be present in the building to receive guaranteed hours and support team continuity

Qualifications

• 1-3 years of experience in occupational therapy — SNF experience a strong fit
• Active Occupational Therapy (OT) license or ability to obtain state licensure
• Comfortable working in a skilled nursing facility environment
• Strong documentation skills and the ability to meet productivity expectations (90% productivity noted)
• Professional appearance and adherence to dress code (black scrubs)
• Ability to work onsite and collaborate with a multidisciplinary team

Knowledge & Skills

• Skilled Nursing Facility (SNF) therapy practices and regulations
• Patient assessment, functional goal setting, and treatment planning
• Therapeutic interventions for mobility, ADLs, cognition, and adaptive equipment
• Clinical documentation and productivity management
• Effective communication and teamwork within a rehab setting
• Familiarity with facility policies and quality standards
