Roles & Responsibilities

• Provide skilled occupational therapy evaluations and treatment for rehab and skilled nursing residents
• Develop individualized treatment plans focused on restoring function and independence
• Implement ADL retraining, adaptive equipment recommendations and therapeutic exercises
• Collaborate with nurses, PT, SLP and case managers to coordinate patient-centered care
• Document assessments, progress notes and discharge planning clearly and promptly
• Educate residents and caregivers on safe home strategies and fall prevention
• Participate in team meetings and contribute to quality improvement efforts

Qualifications

• Current, active Occupational Therapist license (Michigan licensure strongly preferred)
• Experience in rehabilitation and skilled nursing facility settings preferred
• Familiarity with ADL training, adaptive equipment and restorative programs
• Strong documentation skills and comfort using electronic medical records
• Excellent communication and teamwork skills; patient-centered approach
• Current BLS/CPR certification (or willingness to obtain upon hire)

Education

• Bachelor's or Master's degree in Occupational Therapy (OTR/entry-level degree required)

• Adaptive equipment selection and home safety recommendations
• Therapeutic exercise and therapeutic activity prescription
