Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents, prioritizing safety and dignity
Administer medications and treatments accurately and document per facility protocols
Perform thorough patient assessments and report clinical changes promptly
Collaborate with nurses, therapists, and care staff to develop and follow individualized care plans
Assist with activities of daily living and support residents’ comfort and mobility
Maintain clear, timely charting and uphold regulatory and facility documentation standards
Champion a patient-centered approach that improves outcomes and resident satisfaction

Qualifications

Valid LPN license in Pennsylvania (required)
Current BLS/CPR certification (or willingness to obtain upon hire)
Comfortable with medication administration, documentation, and basic nursing procedures
Strong communication skills and a team-oriented mindset
Dependable, professional, and compassionate approach to resident care

Education

Practical Nursing diploma, certificate, or equivalent vocational nursing training

Knowledge & Skills

Medication administration and basic clinical procedures
Patient assessment and vital-sign monitoring
Accurate charting and familiarity with electronic records (EMR) concepts
Infection control, wound care basics, and safety protocols
Time management, prioritization, and teamwork in a fast-paced care setting
