Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents as an LPN
Administer medications and treatments accurately and safely
Monitor clinical status and report changes promptly to the care team
Document assessments, interventions and resident responses in the chart
Collaborate with RNs, CNAs and interdisciplinary team members to support care plans
Support resident independence and dignity through respectful interactions
Assist with admissions, transfers and discharge processes as needed

Qualifications

Current, active LPN license in Pennsylvania (required)
CPR/BLS certification (current) or willingness to obtain
Demonstrated clinical competence in long-term care or similar settings preferred
Strong communication skills and a team-oriented attitude
Ability to pass background checks and meet facility onboarding requirements

Education

LPN diploma or certificate from an accredited nursing program
High school diploma or GED required

Knowledge & Skills

Medication administration and basic wound care knowledge
Familiarity with charting and clinical documentation best practices
Ability to perform resident assessments and recognize changes in condition
Good time management and prioritization skills during busy shifts
Comfort working within an interdisciplinary care team
